Job Description:

EDUCATION
One of the following two options* of ‘A combination of education and experience’.

*A combination of education and experience is defined as:

*Option 1
Graduation from a recognized post-secondary institution with a degree in the study, understanding, or assessment of human behaviour (the degree program must have also included the completion of a practicum or field placement for graduation);
AND
Experience in case management** (minimum 6 months within the last 5 years).

OR

*Option 2
Graduation with a degree from a recognized post-secondary institution;
AND
Experience in case management** (minimum of 1 year within the last 5 years).

** For the purposes of this staffing process, VAC defines 'case management' as:
1. Engagement and relationship building
2. Comprehensive assessment/s
3. Analysis
4. Case planning and consultation
5. Monitoring and evaluation
6. Disengagement

Degree equivalency

EXPERIENCE:
Experience in the delivery of health and social services in an interdisciplinary environment.
Experience in maintaining effective community networks and partnerships.

Knowledge of external health, social and economic services available to Veterans and their families.
Knowledge of health, social and economic issues that impact Veterans and their families.

Asset Qualifications :
Graduation with a degree from a recognized post-secondary institution in social work, nursing, counselling, or occupational therapy.

Degree equivalency

Asset Qualifications :
Experience as a member of the Canadian Armed Forces.
Experience with a military culture.
Experience working at Veterans Affairs Canada.
Experience as a caseworker in a rehabilitation environment.

Operational Requirements:
Case Managers are assigned to a primary work location but may be required to work at various locations and travel between these locations.
Must be willing to occasionally officially represent the Department at functions such as conferences, commemorative events, etc.
